Home Care packages are designed to let you stay in your own home for longer. 

We offer a range of services so you can live independently in your own home for as long as you can.

Once you’re assigned a Home Care Package, we will work with you to develop a personalised care plan that includes the services required to meet your assessed care needs.

  • Can I get a Home Care Package?

    Before you can receive funding for home care services, you’ll need to be assessed.

    Note: If you have already been approved for a home care package or if you’re self-funding your services, you can skip this step.
